Ethereum experienced a notable disruption after a bug in its Prysm client caused validator participation to fall to nearly 75% following the Fusaka upgrade on December 4.
The issue stemmed from a flaw introduced in Prysm PR 15965, which triggered severe resource exhaustion when nodes processed attestations from out-of-sync peers. This forced nodes to recompute prior states, significantly increasing computational load.
According to Ethereum developer Terence Tsao, the bug went unnoticed during testnet trials despite being present weeks before the mainnet upgrade. Over more than 42 epochs, missed slot rates climbed to 18.5%, resulting in validators missing approximately 382 ETH in rewards.
Node operators quickly implemented temporary fixes while the Prysm team rolled out a permanent patch. Although Ethereum recovered without lasting damage, the incident underscored the financial and operational risks tied to consensus-layer instability and the ongoing need for stronger testing frameworks.
